چکیده

Objectives: Rapid and accurate screening for carbapenemase-producing organism (CPOs) in hospitalized patients is critical infection control prevention. The Xpert Carba-R assay designed rapid detection of CPOs, but 1 usually conducted only sample. We evaluated a pooling strategy CPO using the assay. Methods: Swab sets containing 2 swabs were collected from 415 unique at Peking University People’s Hospital. One swab was used test, which 5 different mixed sample treatment solution. prevalence CPOs hospital (5.3%) predicted that 5:1 most economical. As reference method, other tested by culture sequencing. Results: Of samples, 383 negative test 31 positive. All samples Among positive identified strategy, 26 sequencing (including 24 with targeted gene double genes, NDM+/IMP+ VIM+/IMP+), negative. Overall, 198 tests study, 217 saved compared testing individually. efficiency 215%. overall sensitivity (95% CI, 0.840–1), specificity 0.987 0.968–0.995), accuracy 0.970–0.996), predictive value 0.838 0.655–0.939), 0.988–1). Conclusions: showed good potential significantly lower cost.

Multisite Evaluation of Cepheid Xpert Carba-R Assay for Detection of Carbapenemase-Producing Organisms in Rectal Swabs

Rapid identification of patients who are colonized with carbapenemase-producing organisms (CPO) is included in multiple national guidelines for containment of these organisms. In a multisite study, we evaluated the performance of the Cepheid Xpert Carba-R assay, a qualitative diagnostic test that was designed for the rapid detection and differentiation of the blaKPC, blaNDM, blaVIM, blaOXA-48, ...

Comparative evaluation of a prototype chromogenic medium (ChromID CARBA) for detecting carbapenemase-producing Enterobacteriaceae in surveillance rectal swabs.

Carbapenemase-producing Enterobacteriaceae (CPE) are an increasing problem worldwide, and rectal swab surveillance is recommended as a component of infection control programs. The performance of a prototype chromogenic medium (chromID CARBA) was evaluated and compared with media tested by four other screening methods: (i) overnight selective enrichment in 5 ml tryptic soy broth with a 10-μg ert...

Screening rectal swabs for carbapenemase genes.

In an outbreak setting, we screened 16,296 samples from 3,644 patients by PCR for the presence of blaOXA-48, blaVIM, blaIMP, blaNDM, and blaKPC. The blaOXA-48 gene was found in samples from 43 patients infected with 9 different species of Enterobacteriaceae. Five patients had Pseudomonas aeruginosa isolates containing blaVIM. The negative predictive value of screening was 100%, and the positive...

Xpert CARBA-R Assay for the Detection of Carbapenemase-Producing Organisms in Intensive Care Unit Patients of a Korean Tertiary Care Hospital.

Carbapenemase-producing organisms (CPO) are rapidly disseminating worldwide, and their presence in tertiary care hospitals poses a significant threat to the management of nosocomial infections. There is a need to control CPO, especially in intensive care unit (ICU) patients, because these organisms are resistant to most β-lactam antibiotics and are easily transmitted. At present, the identifica...

Multicentre evaluation of the Check-Direct CPE® assay for direct screening of carbapenemase-producing Enterobacteriaceae from rectal swabs.

OBJECTIVES The objective of this study was to evaluate in a multicentre survey the analytical performance of the Check-Direct CPE® assay (CDCPE), a multiplex PCR assay for the detection of carbapenemase-producing Enterobacteriaceae (CPE), directly from rectal swabs. METHODS Adult patients admitted to a high-risk unit in four participating centres were prospectively screened for CPE carriage b...
